• Conduct comprehensive psychosocial assessments for seniors, considering emotional, mental, and social needs.
• Develop personalized care plans in collaboration with clients and their families, focusing on achieving their individual goals and preferences.
• Provide empathetic counseling to clients and their families, helping them navigate difficult life transitions, including end-of-life planning and emotional challenges.
• Advocate for clients’ rights and access to necessary community resources, ensuring they receive thorough support and assistance.
• Collaborate with a multidisciplinary team, including nurses, caregivers, and healthcare professionals, to enhance comprehensive care delivery.
• Monitor clients’ well-being through regular virtual check-ins, assessing living conditions and addressing any concerns promptly.
• Facilitate family meetings to improve communication and resolve conflicts involving caregivers and family members.
• Educate clients and families about available community resources, support services, and programs that can positively impact their lives.
• Maintain detailed and accurate documentation of client interactions, assessments, care plans, and progress notes following ethical and legal standards.
• Stay updated on social work best practices and regulatory requirements to ensure adherence to professional ethics.

In this role, you will primarily work with seniors facing complex health challenges, including chronic and terminal illnesses. Your support will extend to their families, helping them cope with emotional stressors and practical concerns related to caregiving and health transitions. Understanding the unique needs of this population is crucial to delivering effective care and fostering meaningful relationships.

• Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a social work capacity, preferably dealing with geriatric or end-of-life care within a hospice or community setting.
• Strong knowledge of social work principles, aging-related issues, and community resources available for seniors.
• Excellent counseling, assessment, and problem-solving skills, coupled with a proven ability to empathize and connect with clients.
• Advanced communication and interpersonal skills, essential for effective collaboration within interdisciplinary teams.
• Proficiency in using electronic health record (EHR) systems and standard office software applications.

Experience working with hospice patients and their families is a significant advantage. Familiarity with the challenges unique to seniors in hospice care, as well as a background in home visits or telehealth solutions, will enrich your ability to serve our clients effectively.

The Cognitive Care Center offers a competitive salary range of $60,000 to $100,000 annually, commensurate with experience and expertise.
