Home Health Clinical Team Specialist (RN) Full-time or PRN schedules availableRemote (MA Licensed)

We are seeking an experienced Skilled Home Health (SHH) RN who is fluent in the language of home health documentation—someone who has built, corrected, audited, and defended OASIS documentation in real time and understands how it directly impacts compliance, quality, and reimbursement.

This is a remote Clinical Team Specialist (CTS) role with no patient caseload, focused entirely on clinical documentation review and oversight.

What You’ll Do

As a Clinical Team Specialist (RN), you will:

  • Perform detailed OASIS review and validation for accuracy, completeness, and compliance

  • Review, approve, and evaluate 485 Plans of Care for clinical appropriateness

  • Ensure documentation aligns with state, federal, and payer regulations

  • Monitor timely completion and submission of documentation within Homecare Homebase (HCHB)

  • Support direct care teams and clinical coordinators with documentation compliance

  • Participate in QAPI initiatives and quality improvement efforts

  • Help drive consistency, accuracy, and excellence in home health documentation across the organization

What We’re Looking For
  • Active, unrestricted RN license in Massachusetts (required or ability to obtain)

  • Minimum 2+ years of Skilled Home Health (SHH) RN experience

  • Strong, hands-on experience with:

    • OASIS assessments and documentation

    • Homecare Homebase (HCHB)

  • Deep understanding of home health compliance, 485s, and plan-of-care workflows

  • High attention to detail with strong clinical judgment and documentation accuracy

  • Proficiency with Microsoft Office and enterprise systems (Workday experience a plus)

  • Ability to manage structured, detail-heavy clinical review work in a fast-paced environment
