Position Overview

Premire Counseling & Wellness Services is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our innovative team in a full-time remote capacity. As a crucial member within our hospice care environment, you will play a key role in providing therapeutic support and counseling to patients and their families, ensuring effective treatment processes and long-term recovery.

Work Environment

This position offers the flexibility of remote work, allowing you to provide care and support to clients from the comfort of your own home. While your primary duties will be performed remotely, you will maintain close collaboration with colleagues, including the Clinical Director and Executive Director, ensuring a cohesive and therapeutic environment for all clients. Key Responsibilities
  • Conduct comprehensive assessments of client needs, utilizing DSM (Diagnostic and Statistical Manual) criteria to evaluate substance abuse diagnoses.
  • Develop individualized treatment plans that address the unique substance abuse counseling needs of each client, setting appropriate goals based on ASAM (American Society of Addiction Medicine) guidelines.
  • Create discharge and continuum of care plans that facilitate seamless transitions for clients to appropriate subsequent treatment levels.
  • Document all counseling sessions, assessments, treatment updates, and follow-ups accurately in the clinical record, ensuring compliance with organizational standards.
  • Manage a caseload of clients, demonstrating proficiency in case management, treatment planning, and clinical charting.
  • Coordinate support services for clients and their families, facilitating access to necessary resources and documenting these interactions accordingly.
  • Engage and maintain contact with external referral sources, including legal representatives, educational institutions, and employers, to streamline service provision.
  • Utilize a multi-disciplinary team approach to facilitate interventions for clients facing crisis-oriented issues, ensuring effective resolution and support.
Patient Population / Client Focus

You will primarily serve clients dealing with addiction and co-occurring mental health disorders, providing critical therapeutic interventions to support their recovery. Your work will involve engaging with a diverse population, focusing on holistic healing and promoting sustained recovery within a compassionate framework.

Qualifications
  • A Bachelorโ€™s Degree in Social Work from an accredited institution is required; an LCSW certification is mandatory.
  • Exemplary professionalism along with excellent therapeutic boundaries and cultural competency.

Compensation and Benefits
