Job description

HCS Home Care is seeking RNs to join our growing team. We are looking for per diem registered nurses to provide face to face Nursing Visits for our Patients in the boroughs we service, including Brooklyn, Queens, Manhattan, Bronx, Westchester, and Putnam County. HCS Home Care is a Licensed Home Care Agency that is dedicated in providing quality and compassionate care. In accordance with our mission, we are committed to providing the highest level of home care services and focus on meeting the individual needs of our patients and their families in the in the community.

Home Care Registered Nurse Requirements and Qualifications:

  • · Perform Initial Assessment at start of care
  • · Perform Reassessments as authorized
  • · Creating and updating the Plan of Care as appropriate
  • · Perform In Home Supervision of Home Care Aides
  • · Provide Patient and Aide Education as appropriate and necessary
  • · Ability to complete written and electronic documentation appropriately
  • and consistently
  • · Timely submission of paperwork
  • · Organized and Patient oriented
  • · Ability to work independently
  • · Strong verbal and written communication skills
  • · Minimum of 1-2 years clinical nursing experience, Novice RN welcomed
  • · Must accept 20 monthly visits.

· Valid and Current New York State Registered Nurse License and Registration

· Valid and Current Malpractice Insurance

· Physical, PPD and MMR Titers (cannot be older than 6 months)

· 2 typed References

· Social Security Card

SALARY: FEE FOR SERVICE

MUST HAVE OWN CAR/METHOD OF TRANSPORTATION

In-person Visits: $80 to $110 per visit.
