In this role, you will provide essential psychosocial assessments, interventions, and treatment planning for individuals and families facing a variety of challenges stemming from hospitalization, illness, and life situations. • Conduct comprehensive psychosocial assessments to identify the emotional, social, and environmental strengths and challenges our patients face regarding their diagnoses, illnesses, and treatment plans.
• Formulate, develop, and implement individualized treatment plans utilizing appropriate social work interventions tailored to the unique needs of each patient.
• Collaborate closely with healthcare professionals, including physicians and nurses, to develop and implement holistic care plans that include input from patients and their families.
• Facilitate effective discharge planning by coordinating with healthcare providers, third-party payers, and community resources to ensure a smooth transition from inpatient to outpatient care.
• Provide education and counseling to empower patients and families in adapting to challenges related to illness, treatment, and lifestyle changes.
• Utilize various therapeutic approaches including crisis intervention, brief therapy, and long-term counseling, accommodating the specific needs of individuals and families.
• Serve as a resource for other staff members, offering guidance and training as needed to enhance team capabilities in psychosocial support.
• Participate in on-call rotations to assist with after-hours calls, ensuring continuous support for our patients.
• Present at educational conferences to share knowledge and expertise in psychosocial oncology practice, contributing to the field’s body of knowledge.
• Engage in community service and outreach programs aimed at strengthening support networks for cancer survivors and enhancing resource availability.

This role primarily focuses on providing support to patients and families dealing with a spectrum of emotional and psychological challenges associated with chronic illnesses, particularly in the oncology field. You will work with patients facing diagnoses that significantly impact their quality of life and offer strategies to help them navigate their care and recovery journey.

• A Bachelor's Degree in Social Work (BSW) is required; a Master’s Degree in Social Work (MSW) and current LCSW licensure in Colorado are preferred.
• Strong interpersonal skills and a demonstrated ability to communicate effectively with patients, families, and healthcare providers.
• Competence in providing psychosocial assessments and utilizing various therapeutic modalities.
• Ability to work independently as well as collaboratively as part of a remote team.

• Experience working with oncology patients and understanding of psychosocial oncology practices.
• Previous involvement in community health initiatives and education programs.
• Familiarity with remote work technologies and electronic health record systems.

This full-time position offers a competitive salary range of USD 70,000–100,000 per year based on experience and credentials.
