 Home Health RN Care Coordinator

What will I be doing?

  • Participates in the review and oversight of nursing and home health aide services.
  • Review patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans, and discharge.
  • Participates with Director of Home Health in training both new hire and existing qualified personnel.
  • Ensure timely follow-up with patient hospitalizations, order supplies, and communicate with the interdisciplinary team.
  • Perform accurate data entry in electronic medical record.  
  • Collaborate with outside providers and care coordinators to ensure smooth and reliable communication of patient’s plan of care. 
  • Maintain compliance with and stay abreast of changing county, state, and federal regulatory initiatives affecting home health care, including Medicare Conditions of Participation.
  • Understand and comply with policy and procedures set forth by Accurate Home Care.
  • Additional tasks, as assigned by manager.

Education Requirements:

  • Must be licensed as a Registered Nurse (RN) in Minnesota.

Additional Requirements:

  • Maintain confidentiality in accordance with applicable laws and rules.
  • One-year Medicare home health visit nurse experience preferred including proficiency with OASIS.
  • Ability to read, write speak fluently in English.
  • Confirmed negative tuberculosis status.
  • Effective communication, interpersonal and critical thinking skills.
