Case Manager RN — Remote (Michigan)
Contract: 12+ month contract with possible extension
Work Arrangement: 100% remote
License Requirement: Michigan State RN License

1. Mandatory Requirements to Apply

  • Current, active, and unrestricted Michigan Registered Nurse (RN) license (required)
  • Nursing Diploma or Associate Degree in Nursing (required)
  • Bachelor’s Degree in Nursing (strongly preferred)
  • 5+ years clinical nursing experience (acute, post-acute, or community)
  • 2+ year case management experience in a managed care setting (preferred)
  • Experience managing patients via phone, digital platforms, or telehealth (preferred)
  • Intermediate computer proficiency (Microsoft Excel, Outlook, Teams, Word, etc.)
  • Ability to work independently and collaboratively within multidisciplinary teams

2. Roles & Responsibilities

  • Lead coordination of a multidisciplinary care team (social workers, dietitians, pharmacists, physicians, clinical support)
  • Conduct comprehensive patient assessments and develop individualized care plans
  • Engage members/caregivers to identify gaps, address barriers, and improve overall health outcomes
  • Monitor member progress, update care plans, and maintain accurate documentation
  • Provide education on health literacy, medication adherence, nutrition, and self-management
  • Support smooth transitions of care between facilities and home to ensure continuity

3. Pay and Benefits

  • $35-39 per hour
  • 9 days of Paid Time Off
