After-Hours Client Care Coordinator - Remote


RNS Health Care Services Inc. is seeking a highly organized, client-focused After-Hours Client Care Coordinator to join our Coordination team. In this important role, you will help ensure the continuity and reliability of home care services during evenings, weekends, and statutory holidays.

Reporting to the Coordination Supervisor, you will coordinate schedules, respond to service changes, support clients and field staff, and resolve time-sensitive issues. This position is well suited to someone who is calm under pressure, highly organized, solutions-oriented, and comfortable working independently in a fast-paced healthcare environment.


About the Role

As an After-Hours Client Care Coordinator, you will manage and adjust home care schedules outside of regular business hours. You will serve as an important point of contact for clients, healthcare professionals, and referral partners, ensuring that services are appropriately scheduled and that issues are addressed promptly.

The successful candidate will demonstrate sound judgment, strong communication skills, attention to detail, and the ability to prioritize competing demands while maintaining a high standard of client service.


Key Responsibilities

  • Receive and process referrals from Ontario Health atHome, private clients, and other referral sources.
  • Communicate with clients and families to confirm service needs, schedules, and changes.
  • Coordinate and assign appropriate healthcare staff based on client needs, availability, qualifications, and service requirements.
  • Create and update schedules accurately using AlayaCare.
  • Provide healthcare professionals with relevant client information, service details, and travel directions.
  • Monitor schedules and proactively address gaps, cancellations, call-ins, and other service disruptions.
  • Respond to client and staff concerns and resolve scheduling and service issues promptly and professionally.
  • Maintain continuity of care while considering client and staff safety.
  • Escalate complex or unresolved concerns to the Coordination Supervisor and/or Client Services Manager.
  • Complete and maintain required documentation, including incident reports, book-offs, schedule changes, and other records.
  • Maintain accurate and timely communication with internal teams and external partners.
  • Perform other administrative and coordination duties as required.


What You Bring

Education

  • College or university education in healthcare administration, business administration, social services, or a related field is preferred.

Experience

  • Previous experience in community healthcare, home care, scheduling, coordination, dispatch, customer service, or a related environment is an asset.
  • Experience working with healthcare professionals and/or vulnerable populations is considered an asset.

Service Requirements

  • Criminal Record and Judicial Matters Check (CRJMC) completed within the required timeframe.
  • Availability to work varying shifts, including evenings, weekends, and statutory holidays.
  • Availability to work shifts scheduled between 7:00 a.m. and 9:00 p.m.
  • Reliable internet connection and a suitable, private workspace for remote work.
  • Ability to work independently and maintain confidentiality in a remote environment.
