Key Responsibilities

  • Conduct thorough assessments to evaluate clients' social, emotional, and environmental needs, employing a strengths-based perspective.
  • Formulate and implement individualized service plans that articulate specific goals, interventions, and methods for progress tracking.
  • Provide compassionate counseling and crisis support to individuals and families experiencing challenges such as loss, grief, and psychological distress.
  • Facilitate access to essential community resources including financial assistance, housing support, and mental health services.
  • Administer crisis interventions, perform risk assessments, and devise safety plans to protect clients' well-being.
  • Create partnerships with community organizations to enhance service delivery and advocate effectively on behalf of clients.

Patient Population / Client Focus

As a Clinical Social Worker, you will work primarily with individuals and families facing end-of-life issues within our hospice center. Your work will directly support those experiencing grief, depression, and anxiety related to terminal illnesses. You will provide emotional support, facilitate family meetings, and champion the rights and dignity of our clients, ensuring their holistic needs are met with sensitivity and care.

Qualifications

  • Master's degree in Social Work from an accredited program.
  • Active state licensure as a Licensed Certified Social Worker - Clinical (LCSW-C).
  • Solid understanding of trauma-informed care practices and social justice issues affecting marginalized groups.
  • Strong interpersonal and communication skills with a commitment to cultural competence and empathy.
