• Conduct comprehensive psychosocial assessments to evaluate the emotional, social, and environmental factors affecting patients and their families during hospitalization.
• Develop and implement individualized intervention plans utilizing appropriate social work methodologies and therapeutic approaches, including crisis intervention, individual, group, and family therapy.
• Collaborate with healthcare team members, engaging patients and their families in discussions to create actionable plans that meet their unique needs and circumstances.
• Facilitate discharge planning by coordinating with consulting physicians, third-party payers, and various healthcare providers, ensuring that patients receive suitable care upon leaving the detox center.
• Provide ongoing education and counseling to patients and their families to assist them in coping with the challenges presented by their medical conditions.
• Leverage your expertise to educate other healthcare professionals about psychosocial oncology practices and issues related to emotional health.
• Support the community through educational outreach, consultation, and volunteer activities that promote understanding and resources for cancer survivors.
• Participate in the supervision, training, and evaluation of other clinical social work staff, contributing to professional development within the organization.
• Engage in on-call rotation duties to manage after-hour emergency calls and provide timely support when needed.

You will work primarily with patients in a detox center, focusing on those struggling with substance abuse disorders, as well as individuals facing related psychological challenges. Your role is critical in addressing the psychosocial aspects of treatment, helping patients and families navigate the complexities of recovery and post-treatment support.

• A Bachelor’s Degree in Social Work (BSW) required; a Master’s Degree in Social Work (MSW) preferred for advanced practice.
• Current Licensed Clinical Social Worker (LCSW) certification in the state of Georgia.
• Strong understanding of psychosocial assessment and intervention techniques, particularly in a healthcare or detox setting.
• Exceptional communication and interpersonal skills for effective collaboration with patients, families, and other healthcare professionals.
• Proven ability to work independently while effectively managing responsibilities in a remote work environment.

• Experience in substance use disorders, mental health crises, and oncology social work.
• Demonstrated experience in discharge planning and collaborative care coordination.
• Previous involvement in educational programs and community outreach related to mental health.
• Familiarity with electronic health record (EHR) systems for patient documentation and care coordination.

The yearly salary for this position ranges from USD 70,000 to 100,000, commensurate with experience and qualifications.
