As a Licensed Clinical Social Worker at Life Essence Center, your responsibilities will encompass a variety of critical tasks:

• Conduct thorough clinical assessments that evaluate the psychological, social, and emotional needs of patients, ensuring each assessment adheres to established best practices.
• Develop personalized treatment plans utilizing evidence-based interventions such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) to support the mental health needs of clients.
• Maintain accurate and timely clinical records that comply with HIPAA regulations, contributing to unmatched care coordination.
• Collaborate closely with a multidisciplinary team comprising psychiatrists, nurses, and other healthcare professionals to deliver integrated care that addresses every patient’s unique challenges.
• Engage in regular case consultations and collaborative peer reviews focused on enhancing clinical practices and improving patient outcomes.
• Provide education to patients and families regarding mental health issues, treatment pathways, and available resources, empowering them with knowledge and support.
• Stay informed on the latest research developments within the field of mental health, integrating new insights into clinical practices to continually refine and improve treatment approaches.

Your role will focus on serving patients receiving hospice care, many of whom face complex emotional and psychological challenges due to serious illnesses or end-of-life circumstances. You will support a diverse group, addressing individual needs through compassionate and specialized social work interventions.

• A Master’s Degree in Social Work from an accredited institution is required, giving you a strong theoretical foundation to support your practice.
• Valid licensure as a Licensed Clinical Social Worker (LCSW) in Texas, demonstrating your commitment to professional excellence and service.
• Minimum of three years of direct clinical experience in providing psychotherapy services, showcasing your ability to navigate complex patient needs.
• Proficient knowledge of solution-focused therapy interventions that lead to meaningful change in clients’ lives.
• Exceptional communication and interpersonal skills to effectively connect and foster rapport with patients and families.

• Experience working in hospice or palliative care settings, particularly with patients experiencing grief and loss.
