• Conduct thorough clinical assessments for patients enrolled in the hospice program, addressing their emotional, psychological, and social needs.
• Develop, implement, and evaluate individualized treatment plans that incorporate therapeutic interventions geared toward enhancing the quality of life.
• Utilize evidence-based practices and therapeutic techniques, including cognitive-behavioral therapy and mindfulness strategies, to provide emotional support and guidance.
• Maintain accurate and timely documentation in electronic health records (EHR), ensuring adherence to all regulatory and legal requirements.
• Work collaboratively with a multidisciplinary team, including physicians, nurses, and allied health professionals, to coordinate care effectively and ensure comprehensive support for patients and families.
• Provide psychosocial education and resources to patients and their family members, empowering them to navigate the complexities regarding end-of-life issues and bereavement.
• Stay current with research developments and best practices in social work and hospice care to ensure the delivery of high-quality, evidence-based services.

The primary focus of this role is to work with patients facing terminal illnesses and their families. You will be offering critical emotional support during the end-of-life phase, a sensitive and vital time when mental health support can significantly impact patient experiences and family dynamics. Your clinical skills will assist patients in making informed decisions regarding their care while addressing complex emotional responses associated with grief, loss, and terminal transitions.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Texas is mandatory.
• Strong knowledge of psychotherapy interventions, particularly solution-focused and strengths-based approaches.
• Exceptional communication skills, both verbal and written, to foster productive interactions with patients and team members.
• Ability to demonstrate empathy and compassion in challenging situations, particularly concerning end-of-life issues.

• A minimum of two years of clinical experience, particularly in hospice, palliative care, or similar settings.
• Experience collaborating with multidisciplinary teams to promote holistic patient care.
• Familiarity with electronic health records (EHR) systems and documentation standards.

This position offers a competitive annual salary ranging from USD 80,000 to 100,000, commensurate with experience and qualifications.
