• Conduct in-depth clinical assessments of patients to determine their psychosocial needs, emotional challenges, and overall wellbeing, using evidence-based approaches.
• Develop and implement individualized treatment plans that incorporate solution-focused interventions, ensuring alignment with each patient’s goals and values.
• Facilitate therapeutic sessions through various modalities, including cognitive-behavioral therapy and supportive counseling, especially designed for a hospice setting.
• Maintain comprehensive and timely documentation of clinical notes, treatment plans, and progress reports, ensuring compliance with legal and ethical standards.
• Collaborate with a multidisciplinary team, including physicians, nurses, and chaplains, to coordinate and enhance holistic patient care.
• Participate actively in case consultations and peer review meetings to improve clinical practices and patient outcomes.
• Educate and support patients and their families about mental health concepts, available resources, and coping strategies, empowering them throughout the hospice journey.
• Stay updated on the latest research and best practices in palliative care and social work to refine therapeutic approaches continuously.

The patients you will serve at our hospice facility come from diverse backgrounds and possess a variety of complex emotional needs as they face end-of-life circumstances. You will work with individuals and families navigating grief, anxiety, and existential questions, providing compassionate support tailored to their unique experiences. • A Master’s degree in Social Work from an accredited institution is required.
• A valid Licensed Clinical Social Worker (LCSW) license in Virginia.
• At least two years of clinical experience in a healthcare setting, with a focus on hospice or palliative care preferred.
• Proven expertise in employing solution-focused therapy and psychotherapeutic interventions.
• Exceptional communication and interpersonal skills, with an ability to build rapport and trust with patients and their families.
