As a Licensed Professional Counselor, you will:

• Conduct comprehensive assessments to identify the unique needs of each patient and create tailored care plans.
• Facilitate consistent communication with supervisors, direct care staff, family members, and other involved parties about patient progress and treatment outcomes.
• Develop a structured schedule of appointments, home visits, and observations to monitor patient needs and progress effectively.
• Coordinate in-home visits for evaluations, chart reviews, and data analyses to ensure holistic care delivery.
• Implement and oversee care plans to guarantee they are appropriate and effective, adapting as necessary based on patient progress.
• Engage in Interdisciplinary Team Meetings, contributing insights and recommendations based on clinical findings.
• Document and maintain accurate monthly and quarterly progress notes to track patient development and care interventions.
• Provide training and mentorship to colleagues to ensure they are equipped to implement care plans and maintain proper documentation.
• Conduct follow-up interviews and observations to provide ongoing support tailored to individual patient needs.
• Act as a liaison within the mental health community, advocating for patient needs and promoting developmental goals.
• Assist in creating effective training materials aimed at enhancing program quality and service delivery for individuals in care.

Your work will primarily involve interacting with a diverse range of clients, including both adults and youth. This position allows you to engage with various populations facing mental health challenges, including those dealing with family dynamics, trauma, substance misuse, and other psychological concerns. Your role will be vital in fostering relationships built on trust and respect, crucial for effective therapy outcomes.

The ideal candidate will possess the following qualifications:

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.
• Active and unrestricted licensure as a Professional Counselor (LPC) in Pennsylvania.
• A minimum of 2 years of professional counseling experience in a clinical setting.
• Specialization in areas such as family therapy, couples therapy, substance misuse, or trauma is highly preferred.
• A strong understanding of legal and ethical standards governing mental health practice.
• Exceptional interpersonal and communication skills, with the ability to connect with clients from diverse backgrounds.
• Demonstrated ability to work collaboratively within a healthcare team and contribute to positive patient outcomes.
• Proficiency in electronic medical records (EMR) software and familiarity with related technologies.

While the minimum requirement is 2 years of counseling experience, candidates with a background in evidence-based practices, crisis intervention, or innovative therapy techniques will be viewed favorably. Experience working within community health settings may also enhance your application.
