• Conduct comprehensive assessments to identify clients' social, emotional, and environmental challenges, leading to individualized service plans.
• Develop and implement structured service plans with clear goals, interventions, and ongoing progress monitoring tailored to each client's situation.
• Provide virtual counseling sessions and telehealth support to individuals and families experiencing challenges such as mental health issues, trauma, and life transitions.
• Engage actively with clients to facilitate personalized service plans, ensuring they have access to necessary community resources.
• Deliver crisis intervention and support, performing risk assessments and safety planning when needed.
• Foster collaborative relationships with community agencies and organizations to enhance service coordination and advocate effectively for clients.
• Take part in community outreach initiatives aimed at raising awareness about available services and promoting mental health support.
• Participate in regular team meetings, case conferences, and professional development training to stay informed on best practices and agency policies.

In this role, you will primarily support individuals facing significant social challenges, including homelessness, substance abuse, domestic violence, and poverty. • Bachelor's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• Exceptional communication skills and cultural competence to effectively engage diverse client populations.
• Proficient problem-solving abilities and resilience in handling complex cases and crises.

• At least 2 years of experience working in a social service agency or similar environment.
• Experience in telehealth or remote counseling services is highly advantageous.
• Familiarity with evidence-based practices in mental health counseling and case management.

The position offers a competitive salary range of USD 60,000 to 100,000 annually, commensurate with experience and qualifications. In addition, successful candidates will receive a sign-on bonus as part of their compensation package.
