This position is fully remote, providing flexibility while still requiring the LCSW to be available for on-campus consultations as needed. Our Hospice Center is dedicated to compassionate end-of-life care, and our multidisciplinary team collaborates closely to provide seamless support to patients and their families, addressing the complex needs typical in hospice care settings.

• Independently manage a designated caseload of patients by performing comprehensive evaluations and assessments to identify the social, emotional, financial, and cultural factors that influence patient care.
• Develop and implement individualized treatment plans that address psychosocial barriers to care, ensuring safe and timely transitions throughout the continuum of care.
• Provide crisis intervention services, guiding patients and families through emotionally charged moments while offering support and resources.
• Lead team meetings and initiatives to address advance directives and life care decision-making, empowering patients to take an active role in their treatment plans.
• Collaborate with case managers and other healthcare professionals to provide education and resources to families, ensuring that they receive holistic support tailored to their circumstances.
• Supervise and mentor junior social work staff, interns, and volunteers, fostering a culture of learning and growth within the department.
• Engage with community resources and agencies to facilitate access to necessary services, mitigating potential readmissions and enhancing care continuity.
• Maintain accurate documentation of psychosocial services rendered and treatment plans in compliance with legal and organizational standards.
• Participate actively in consultations and interdisciplinary team rounds, contributing valuable inputs based on caseload assignments.
• Available for on-call duties on a rotating basis to provide immediate support for crisis situations that may arise.

The LCSW will work primarily with patients in hospice care, many of whom are facing terminal illnesses. • Current licensure as a Licensed Clinical Social Worker (LCSW) in California.
• A Bachelor's Degree in Social Work (BSW) from an accredited institution; a Master's Degree in Social Work (MSW) is preferred.
• A minimum of two years of relevant experience in healthcare or a similar social work setting, with a focus on patient-centered care.
• Demonstrated ability to handle complex social cases, utilizing evidence-based interventions to enhance patient outcomes.
• Strong communication skills and the ability to work collaboratively with diverse teams.

• Experience working in hospice or palliative care settings.
• Familiarity with community resources and programs relevant to end-of-life care.
• Previous exposure to case management and healthcare regulations.
• Proven crisis intervention experience, particularly with terminally ill patients and their families.

This full-time position offers a competitive salary range of USD 120,000 – 130,000 annually.
