Remote RN Case Manager (Texas Residents Only)

Are you an experienced Registered Nurse looking to transition from bedside care into a role focused on care coordination, member advocacy, and improving health outcomes? This fully remote opportunity allows you to support high-risk populations while maintaining a strong work-life balance.

Required Qualifications
  • Active, unrestricted Texas RN license in good standing
  • Must reside in Texas
  • 3+ years of experience in community health, public health, chronic disease management, community nursing, or case management
  • 1+ year of experience in care coordination and disease management/patient education
  • 3+ years coordinating care with primary care providers
  • Outpatient case management experience within a health insurance, payer, or managed care environment
Preferred Qualifications
  • Medicaid Managed Care experience (STAR, CHIP)
  • Marketplace or Commercial Health Plan experience
  • Knowledge of NCQA, HEDIS, HHSC, and TDI standards
  • CCM or other healthcare certifications
  • Experience with Epic or other EHR systems
Key Responsibilities
  • Conduct telephonic assessments for high-risk members

Pay and Benefits
