UW Medical Center - Montlake has an outstanding opportunity for an experienced Registered Nurse to join our Digestive Health Center clinic.
POSITION DESCRIPTION
As a key member of the Inflammatory Bowel Disease (IBD) team, the Care Management RN plays a vital role in delivering high-quality, patient-centered care to individuals with complex needs. This role provides clinical triage; monitors laboratory, imaging, and procedural results; manages medication regimens; and coordinates prior authorizations to ensure timely access to treatment.
The Care Management RN collaborates closely with an interdisciplinary healthcare team to support care coordination, ensure clear communication of the plan of care, and promote alignment across disciplines. Through proactive care management and close patient follow-up, this role also supports efforts to reduce avoidable hospital admissions and readmissions. This position is instrumental in delivering safe, efficient, and seamless care, with a focus on optimizing patient outcomes and the overall patient experience.
WORK SCHEDULE
  • Monday-Friday, 8-hour shifts
  • Days

PRIMARY JOB RESPONSIBILITIES
  • Direct and indirect patient care in the Digestive Health Center
  • Communicate with physicians about changes in patient's clinical condition including hemodynamic monitoring and symptomatology
  • Respond quickly and accurately to changes in the patient's condition

REQUIRED POSITION QUALIFICATIONS
  • 2 years of current/recent med-surg nursing experience required
  • Care management experience preferred
  • Current license to practice as a registered nurse in the State of Washington
