Care Coordinator - Remote/Hybrid
Key Responsibilities:
Essential Functions
The Care Coordinator works under the direct supervision of the Manager of Chronic Care with responsibility to the Director of Operations. The Coordinator conducts patient outreach and is responsible for monitoring patient health status, review patients' conditions and symptoms, evaluation of patients' compliance with physician directed care, and orchestrating seamless care for patients.
  • Demonstrates, through behavior, Center for Primary Care's Mission Statement.
  • Takes responsibility for providing exceptional customer service.
  • Maintains patient confidentiality at all times' adheres to all HIPAA guidelines.
  • Follows CPC guidelines, infection prevention practices, demonstrates appropriate use of PPE, performs hand washing according to polity, and follows safety and workplace incident procedures.
  • Performs miscellaneous job-related duties as assigned.
  • Provides an effective communication link between patients, medical staff, and other providers.
  • Performs timely follow up calls and collects health information through patient outreach.
  • Evaluates areas of concern, and assist patients with their health care needs, including medical record retrieval, psychosocial support, community resources, medication assistance, etc.
  • Provides ongoing education and outreach to support patient's achievement of self-management.
  • Coordinate and collaborates with medical staff involved in the care of the patient; ensuring all preventative health opportunities are discussed and conducted, as appropriate.
  • Assist in coordination of care with primary care physicians, specialists, patients, insurance companies, and other providers in the community.
  • Maintains accurate and complete documentation within the EMR, and other electronic databases.
  • Meets with office staff monthly to provide updates.
  • Reports information regarding quality improvement activities for any care gap closure.

Requirements
Qualifications for Success:
Education, License/Certification, and Experience Requirements
Education:High School graduate or GED required.
Licensure/Certification: Registered MA, Certified MA, or current LPN required. Basic Life Support or be able to obtain within 6 months.
Experience: 1-2 years of Medical Assistant or similar medical experience.
  • Strong working medical knowledge of the clinical needs and disease processes for the chronically ill population.
  • Knowledge of general health care practices, office workflows, and medical terminology.
  • Must have good verbal and written communication skills.
  • Ability to work autonomously and be directly accountable for assignments.
  • Strong computer and EMR skills.

Additional Job Details:
Work Setting: Remote/hybrid. This position may require limited travel throughout the business day.
Job Type: Full-Time
Schedule: Weekdays and ability to work flexible and extended hours, as needed.
Compensation: Market competitive base pay, commensurate with education and experience.
