• Conduct thorough psychosocial assessments to understand patients' and families' emotional needs, strengths, and coping resources, while documenting findings in a secure and timely manner.
• Facilitate individual and family therapy sessions, applying evidence-based therapeutic approaches to help clients navigate grief, loss, and other complex emotions associated with hospice care.
• Develop, implement, and continually evaluate individualized treatment plans that focus on specific, measurable goals addressing both psychological and social aspects of care.
• Collaborate closely with an interdisciplinary team of healthcare professionals, including nurses, physicians, and chaplains, to ensure a cohesive approach to patient-centered care.
• Provide crisis intervention services, conduct risk assessments, and facilitate safety planning to support families during acute emotional distress.
• Lead psychoeducation sessions, support groups, and community outreach initiatives focused on bereavement and the importance of mental health in palliative care.
• Maintain compliance with ethical and legal standards in documentation and service delivery, ensuring patient confidentiality and data security.

The ideal candidate will work with patients of all ages facing end-of-life challenges, as well as their families. This role requires sensitivity and an understanding of the unique psychological and social issues that arise in hospice settings, including anticipatory grief, loss, and navigating complex family dynamics.

• Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Florida.
• At least two years of clinical experience in mental health or outpatient settings, preferably in chronic illness or hospice environments.
• Demonstrated knowledge of evidence-based therapeutic modalities, particularly in end-of-life care and bereavement.
• Strong interpersonal skills, with the ability to effectively communicate and build rapport with patients, families, and multidisciplinary teams.

• Experience in hospice care or palliative services.
• Familiarity with electronic health record (EHR) systems, telehealth platforms, and documentation best practices.
• Skills in culturally sensitive counseling techniques suitable for a diverse patient population.
