Woodlands Primary Healthcare is a growing primary care practice seeking a detail-oriented, self-motivated Medical Assistant to support our Chronic Care Management (CCM) and Principal Care Management (PCM) programs remotely. This is an excellent opportunity for a bilingual MA who thrives in a patient-centered environment and is comfortable working independently from home.
About the Role

You will serve as a key touchpoint for our chronic disease patients — conducting monthly care coordination calls, documenting clinical interactions, updating care plans, and ensuring patients remain engaged with their treatment goals between office visits. This is a remote position requiring reliable internet, a quiet workspace, and strong self-discipline.
Key Responsibilities

  • Conduct monthly CCM/PCM outreach calls with enrolled patients to review symptoms, medications, and care plan progress
  • Accurately document all encounters and time logs in eClinicalWorks (ECW)
  • Coordinate care between specialists, PCP, and ancillary providers
  • Assist with care plan creation and updates under provider supervision
  • Identify and escalate urgent patient concerns to the clinical team
  • Maintain compliance with CMS billing requirements for CCM/PCM time tracking
  • Support patient enrollment and consent documentation
Requirements

  • Medical Assistant certification (CMA, RMA, or equivalent) preferred
  • Minimum 2–3 years of experience in a clinical or care coordination role
  • Proficiency in eClinicalWorks (ECW) — required, no exceptions
  • Experience with CCM, PCM, or RPM programs strongly preferred
  • Bilingual — English/Spanish required (spoken and written)
  • Must reside in the Spring, TX or Katy, TX area
  • Reliable home office setup with stable internet connection
  • Strong communication, documentation, and time-management skills
  • Ability to work independently with minimal supervision
Compensation & Schedule

  • Pay: $17–$25/hour depending on experience
