As a Licensed Professional Counselor at Mental Thrive, you will engage in a variety of tasks aimed at supporting patients during their mental health journey. Your day-to-day responsibilities will include:

• Conducting comprehensive assessments to identify the unique needs of each patient, facilitating tailored treatment plans that evolve based on ongoing evaluations.
• Maintaining clear and effective communication with supervisors, family members, and other stakeholders, ensuring a well-rounded understanding of each patient’s progress.
• Creating and managing a schedule that includes appointments, home visits, and observational meetings to monitor patient advancements and provide targeted interventions.
• Coordinating home visits to conduct in-depth observations, chart reviews, and data analysis that inform your therapeutic strategies.
• Assuring that the implementation of treatment plans is executed effectively, adapting strategies as necessary to accommodate patient feedback and progress.
• Actively participating in interdisciplinary team meetings to collaborate on patient care and sharing insights that may enhance overall service delivery.
• Documenting monthly and quarterly progress notes that reflect the dynamics of patient interactions and treatment outcomes, ensuring compliance with organizational standards and practices.
• Facilitating training sessions for staff to ensure competency in executing program plans and documenting patient progress effectively.
• Following up with patients through interviews, home visits, and ongoing observations to ensure continued support based on their evolving needs.
• Serving as a liaison to the community, collaborating with other mental health professionals and support systems to foster a robust network for patient support.

You will work with a diverse range of clients, including both adults and youth facing various mental health challenges. Emphasizing a holistic approach, your expertise may encompass areas such as family dynamics, trauma, substance use disorders, and other psychological concerns that impact emotional well-being.

The ideal candidate will possess the following qualifications:

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.
• An active and unrestricted license as a Professional Counselor (LPC) in the state of Georgia.
• A minimum of two years of counseling experience, preferably within a clinical setting.
• A strong understanding of legal and ethical guidelines relevant to mental health practices.
• Excellent verbal and written communication skills, demonstrating the ability to connect with a varied clientele.
• A proven capability to collaborate effectively with a healthcare team, fostering a supportive environment for patient care.
• Familiarity with Electronic Medical Records (EMR) software and related documentation tools is advantageous.
