Job description

A specialized Medication Therapy Management (MTM) Pharmacist opportunity is available, offering a 13-week contract with the potential for direct hire. This role is designed for pharmacists experienced in specialty MTM environments, providing critical pharmaceutical care either onsite or remotely. The work location is Denham Springs, LA, but remote arrangements can be considered for qualified candidates holding the required licensure.

Key Responsibilities:

  • Prepare and dispense medications per physician orders, adhering to established protocols and policies.
  • Accurately interpret and transcribe verbal and written prescriptions into computerized patient medication profiles.
  • Maintain detailed and current pharmacy patient medication profiles.
  • Dispense pharmaceuticals and controlled substances responsibly, complying with legal requirements and documentation standards.
  • Ensure patient drug therapies are safe, effective, and cost-efficient by monitoring for contraindications, interactions, allergies, and appropriate dosing.
  • Detect and report adverse drug reactions promptly and accurately.
  • Provide clinical consultations to healthcare providers, recommending cost-effective therapeutic alternatives as appropriate.
  • Document clinical activities comprehensively and participate in quality improvement initiatives including medication reviews, audits, and compliance activities.
  • Supervise pharmacy support staff, oversee daily pharmacy technician activities, and assist with performance evaluations.
  • Maintain timely pharmacy services through effective workflow organization and independent work practices.
  • Conduct medication storage inspections, replacing expired or unusable medications.
  • Professionally handle communications through phone and direct patient interactions.

Qualifications and Desired Experience:

  • Current Louisiana Pharmacist License mandatory.
  • Licensure in a minimum of five additional states from the designated list including Alaska, Arkansas, California, District of Columbia, Delaware, Hawaii, Indiana, Maine, Maryland, Massachusetts, Montana, New Hampshire, North Dakota, Oklahoma, South Dakota, Utah, Vermont, Virginia, West Virginia, Wyoming.
  • Commitment to obtain licensure in at least 10 additional states within 18 months.
  • Prior experience in retail or hospital pharmacy settings is required.
  • Proven ability to work independently with minimal supervision while managing multiple tasks efficiently.
