• Lead psychoeducational and process-oriented therapy groups tailored to meet the unique needs of patients.
• Begin discharge planning on the day of admission for all patients, utilizing a patient-centered approach.
• Conduct individual therapy sessions to assess mental health needs and develop personalized treatment plans.
• Complete discharge sessions with patients to ensure they understand coping skills, their transition plan, and available resources.
• Confirm discharge appointments with patients and their support systems to facilitate a smooth transition.
• Communicate effectively with referral sources and outpatient providers to maintain continuity of care throughout the patient’s journey.
• Facilitate transfers for patients needing higher levels of care, collaborating closely with healthcare teams.
• Respond to crises in sessions with a calm and effective strategy, ensuring patient and staff safety.
• Support milieu staff by providing guidance and assistance in patient interactions and care processes.
• Actively participate in clinical outreach and program tours to increase community awareness of services.
• Assess potential suicidality and the risk of harm to self or others, taking immediate and appropriate action as necessary.
• Collaborate with members of the clinical team, providing additional support as needed in their absence.
• Work with the Utilization Review Department to ensure proper authorization and payment processing for services rendered.

This position will involve working with a diverse range of clients, including children, adolescents, and adults from various backgrounds. • A Master’s degree in Social Work, Counseling, Psychology, or another related mental health discipline is required.
• Must be independently licensed as a Licensed Clinical Social Worker (LCSW).

• Proven experience in completing detailed documentation of services and assessments in a timely manner.

• Annual salary ranging from $120,000 to $130,000 for full-time employment, reflecting your experience and expertise in the field.
