  • Confirm discharge appointments with patients and their support systems to facilitate a smooth transition.
  • Communicate effectively with referral sources and outpatient providers to maintain continuity of care throughout the patientโ€™s journey.
  • Facilitate transfers for patients needing higher levels of care, collaborating closely with healthcare teams.
  • Respond to crises in sessions with a calm and effective strategy, ensuring patient and staff safety.
  • Support milieu staff by providing guidance and assistance in patient interactions and care processes.
  • Actively participate in clinical outreach and program tours to increase community awareness of services.
