Key Responsibilities

In your role as an LCSW, you will undertake the following key responsibilities:

• Conduct in-depth assessments of seniors’ psychosocial needs, encompassing emotional, mental, and social factors.
• Collaborate closely with clients and their families to develop personalized care plans tailored to promote their specific goals and preferences.
• Offer empathetic counseling and emotional support to clients, assisting them in navigating life transitions, such as relocation to assisted living, the loss of a spouse, or health issues.
• Advocate for clients’ rights, ensuring they have access to necessary resources and community support services.
• Facilitate effective communication among family members involved in the client's care to promote understanding and collaboration.
• Conduct regular check-ins to assess clients' living conditions and monitor their overall well-being, addressing any emergent concerns promptly.
• Educate clients and their families about available resources, support groups, and community programs to enhance their engagement and access to services.
• Maintain thorough and accurate documentation of client interactions, assessments, and care plans to ensure continuity of care and compliance with regulatory standards.
• Stay current on social work practices and regulations, incorporating best practices into your work.

Patient Population / Client Focus

The primary focus of this role is to serve the senior population residing in North Bergen, New Jersey. You will work with a diverse group of clients facing various challenges, including mental health issues, social isolation, and age-related illnesses. Your empathetic approach and understanding of the unique hurdles seniors encounter will be crucial in fostering a positive and empowering environment.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey.
• A minimum of two years of relevant experience in a social work capacity, preferably within a community setting or working with older adults.
• Strong understanding of social work principles, aging-related issues, and available community resources.
• Excellent assessment, counseling, and problem-solving capabilities.
• Demonstrated ability to empathize with seniors and their families and to build trusting relationships.

• Familiarity with electronic health record (EHR) systems and MS Office applications.

Compensation and Benefits
