Serene Reflections Therapy is seeking a dedicated and experienced Licensed Master Social Worker (LMSW) to join our team in Allentown, Pennsylvania. This position offers a remote work setting, allowing for flexible scheduling within a compassionate and proactive team. You will work from home while conducting regular visits to a hospice center and clients' residences as needed. You will be expected to maintain open lines of communication with clients, their families, and our healthcare professionals, ensuring a seamless care experience for all involved.

As a Licensed Master Social Worker, your role will include:

• Conducting thorough assessments of seniors' psychosocial needs, considering factors such as emotional health, family dynamics, and social engagement.
• Collaborating with clients and their families to create personalized care plans that reflect their unique goals, preferences, and health status.
• Providing counseling and emotional support to seniors navigating life transitions—such as changing health status, loss of independence, or relocation.
• Advocating for clients' rights and facilitating access to essential community services and resources, ensuring they receive the comprehensive support they need.
• Coordinating with interdisciplinary teams including nurses, caregivers, and healthcare professionals to enhance the overall care experience.
• Conducting regular check-ins with clients to monitor their well-being, assess living conditions, and promptly address any emerging concerns.
• Facilitating effective communication among family members to resolve conflicts, strengthen relationships, and improve care coordination.
• Providing educational resources to clients and families regarding available community support and engagement opportunities.
• Maintaining detailed and accurate documentation of client interactions, assessments, and care plans using electronic health record (EHR) systems.
• Staying abreast of current social work practices, policies, and ethical standards to ensure compliance with best practices in the field.

Your primary focus will be on elderly clients and their families, including those in hospice care. This role demands empathy and the ability to understand the diverse challenges they face, ensuring you provide not only support but also interventions that enhance their overall quality of life.

• Master's degree in Social Work from an accredited institution.
• Valid licensure as a Licensed Master Social Worker (LMSW) in Pennsylvania.
• A minimum of two years of experience in a social work role, particularly within geriatric services or community health settings.
• Comprehensive knowledge of social work principles, aging-related issues, and community resources.
• Exceptional assessment, counseling, and problem-solving skills.

Experience working with hospice clients or in a community center specializing in senior care is highly desirable. Familiarity with the complexities of geriatric care plans, interdisciplinary team dynamics, and community resource navigation will give candidates a distinct advantage.
