This position is primarily remote, allowing for flexibility in your schedule while maintaining a strong presence in the community. You will collaborate with a multidisciplinary team of healthcare professionals, including nurses and caregivers, to coordinate care and develop comprehensive service plans. You will also have occasional face-to-face meetings with clients and their families when necessary, and may need to attend community events to strengthen our outreach efforts.

• Conduct detailed assessments of seniors' psychosocial needs, using tailored evaluation methods to consider emotional, mental, and social factors.
• Develop individualized care plans in collaboration with clients and their families, aligning their goals and preferences with the available resources.
• Provide counseling and emotional support to seniors and their families, guiding them through transitions and significant life changes such as loss or relocation.
• Advocate for clients' rights and navigate them through community resources, ensuring access to essential services such as healthcare, housing, and financial assistance.
• Work closely with interdisciplinary teams to enhance service delivery, ensuring that all team members are aware of each client's unique needs and circumstances.
• Conduct regular monitoring visits to clients' homes to assess their living situations and provide updates on their well-being, addressing any concerns as they arise.
• Facilitate communication among family members involved in the care of seniors to mitigate conflicts and clarify roles, ensuring a cohesive support system.
• Educate clients and their families about available community resources and participation opportunities, empowering them to engage actively with their community.
• Maintain accurate and timely documentation of client assessments, care plans, interactions, and progress notes, utilizing electronic health record (EHR) systems effectively.
• Stay updated with best practices in social work, policies, and compliance regulations to uphold the ethical standards of practice.

This role will focus primarily on seniors and their families, understanding their unique challenges, including health issues, social isolation, and the complexities of aging. As a Social Worker at Bright Path, you will be instrumental in facilitating connections between clients and essential services, thereby significantly contributing to their overall wellness and social engagement.

• Master's degree in Social Work from an accredited institution.
• Valid and current state licensure as a Licensed Master Social Worker (LMSW).
• At least two years of relevant experience in a similar social work role, particularly in a community health or geriatric setting.
• Deep understanding of social work principles, aging-related issues, and knowledge of available community resources.
• Excels in assessment, counseling techniques, and problem-solving abilities.
• Strong empathetic communication skills, enabling the establishment of rapport with clients and interdisciplinary teams.
• Familiarity with electronic health records (EHR) and related computer applications.

• Experience in community outreach programs or initiatives specifically targeting senior populations.
• Proven track record of advocating for clients and navigating complex social services systems.
