Position Overview

We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team at the Cognitive Care Center in Chicago, Illinois. This full-time remote position presents a unique opportunity to significantly enhance the lives of seniors by providing expert psychosocial support and counseling services. Work Environment

This role is conducted remotely, allowing for flexibility while maintaining a structured schedule to support our senior clients effectively. The Social Worker will primarily work from home, with occasional requirements for onsite visits, community engagements, or team meetings as needed. Work hours are structured from Monday to Friday, 8:00 AM to 4:30 PM, with potential adjustments for client needs, including evening or weekend hours. This position requires on-call availability to respond to urgent client situations as necessary.

Key Responsibilities
  • Conduct thorough assessments of seniors' psychosocial needs through interviews and observations to determine appropriate intervention strategies.
  • Collaborate with clients and their families to develop comprehensive, personalized care plans targeting their specific emotional and social goals.
  • Provide counseling and emotional support to help seniors and their families navigate transitions related to aging, including health changes, loss, or relocation.
  • Advocate passionately for clients’ rights and guide them to crucial community resources such as healthcare services, support groups, and financial assistance.
  • Work closely with an interdisciplinary team, including healthcare professionals, occupational therapists, and community service representatives, to ensure holistic client care.
  • Initiate regular check-ins with clients to monitor progress, assess well-being, and make necessary adjustments to care plans in response to evolving needs.
  • Educate clients and family members about available resources and support networks to foster social engagement and enhance quality of life.
  • Document all client interactions, assessments, care plans, and outcomes meticulously to ensure compliance with ethical standards and organizational policies.
  • Stay current with best practices, policies, and regulations in social work to uphold the highest ethical standards and practices in service delivery.
Patient Population / Client Focus

This position focuses primarily on seniors who may be experiencing a range of challenges associated with aging, including social isolation, mental health issues, and changes in physical health. The LCSW will be instrumental in building trusting relationships with clients, allowing for open communication about their needs and concerns. Qualifications
  • A Master's degree in Social Work from an accredited program is required for this role.
