• Develop and implement personalized treatment plans that specifically address each client’s mental health and substance use needs, ensuring the integration of evidence-based therapeutic approaches.
• Conduct individual therapy sessions and facilitate group therapy meetings, creating a safe space for clients to share their experiences and challenges while maintaining strict confidentiality and privacy.
• Participate actively in case conferences and team meetings to collaborate on client care, sharing insights and strategies to enhance treatment outcomes.
• Perform thorough assessments by analyzing gathered information to determine appropriate diagnoses, recommend treatment plans, and identify the need for referrals to other specialists or services.
• Create and lead engaging therapeutic activities, such as group discussions and recreational exercises, to enhance coping mechanisms and foster community among clients.
• Employ coaching models and natural environment teaching methods to support client engagement and life skills development.

The clientele that you will serve comprises individuals grappling with mental health disorders and substance use issues within hospice care. Our clients are often navigating complex emotional landscapes, and you will help them address their unique challenges by implementing tailored therapeutic strategies.

• A Master’s degree in Social Work, Counseling, or a closely related field is required to ensure a strong foundation of knowledge and skills.
• A current, valid, and unrestricted license as a Licensed Clinical Social Worker (LCSW) in the state is essential for practice.
• Proven organizational and prioritization abilities, including effective time management skills, are necessary to manage a caseload and meet the diverse needs of clients.
• Demonstrated willingness for self-reflection and engagement in ongoing supervision and professional development.

• A minimum of 3 years of post-licensure experience in a mental health or substance use setting is preferred, showcasing your ability to handle complex situations and varied client needs.
• Experience working within hospice care or similar settings is a plus, as it provides insights into the unique emotional and psychosocial considerations of these clients.

This position offers an attractive annual salary range of USD 100,000 - 105,000, along with a comprehensive benefits package that includes:
