Key Responsibilities
  • Conduct comprehensive and ongoing assessments of patients to tailor therapeutic approaches that address individual needs.
  • Facilitate thorough communication between supervisors, families, and healthcare team members to ensure continuity of care and track patient progress.
  • Develop and propose a structured schedule of appointments, home visits, and observations to effectively monitor patient advancement.
  • Coordinate and conduct in-home visits for assessments and observations, utilizing data analysis to inform treatment plans.
  • Ensure the rigorous implementation of established program plans, adapting strategies in collaboration with the multidisciplinary team as needed.
  • Participate actively in Interdisciplinary Team Meetings, contributing insights and recommendations based on patient interactions and outcomes.
  • Document monthly and quarterly progress notes to maintain accurate and accessible patient records, fostering transparency in treatment processes.
  • Organize training sessions to enhance the effectiveness of team members in implementing program plans and documentation practices.
  • Conduct follow-up interviews, home visits, and observations to provide continued support and assess ongoing needs of individuals.
  • Act as an essential liaison with the greater mental health community, advocating for patient needs and helping achieve developmental goals.
  • Contribute to the creation and refinement of training materials aimed at improving individualized care programs.
Patient Population / Client Focus

This role focuses on serving a diverse clientele comprising youth and adults grappling with various mental health challenges. The ideal candidate will demonstrate an ability to engage with different populations, tailoring their approach to suit the unique backgrounds and circumstances of each client. Qualifications
  • A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

  • Candidates with specializations in family therapy, couples therapy, substance misuse recovery, or trauma counseling will be given priority.
