Key Responsibilities
  • Conduct detailed evaluations and assessments to understand each child's unique needs and development trajectory.
  • Participate in the development and implementation of Individualized Family Service Plans (IFSP), tailoring interventions to meet specific family dynamics and child development goals.
  • Regularly monitor and evaluate outcomes, collaborating with a multidisciplinary team to ensure comprehensive service delivery.
  • Provide essential support to families, offering guidance and resources that promote the emotional and developmental growth of their children.
  • Ensure compliance with all relevant Federal and State regulations governing social work practice in hospice settings.

Patient Population / Client Focus

In this role, you will primarily work with children under three years of age who have disabilities or developmental delays. Qualifications
