We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) for a full-time remote position within our hospice services at Phoenix Psychotherapy Counseling Center. In this critical role, you will work with our team to support individuals and families facing end-of-life challenges, helping them navigate emotional, social, and practical matters associated with hospice care.

Key Responsibilities
  • Conduct comprehensive evaluations and assessments of patients and their families to understand their needs and challenges.
  • Engage actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring each plan aligns with the familyโ€™s goals and preferences.
  • Monitor progress and outcomes as a vital member of the multidisciplinary team, adjusting service plans as necessary to enhance patient wellbeing.
  • Provide emotional support and practical assistance to families dealing with grief and loss, employing evidence-based interventions tailored to their unique circumstances.
  • Ensure compliance with all relevant federal and state regulations and agency policies, maintaining the highest ethical standards of practice.
  • Utilize the coaching model and natural environment teaching approaches to empower families, equipping them with tools and knowledge to support their loved ones.
  • Establish strong relationships with children, families, and colleagues to cultivate a nurturing and supportive environment conducive to healing.
Patient Population / Client Focus

Our social workers primarily support patients receiving hospice care, often individuals with life-limiting conditions, and their families. You will work with diverse populations, addressing a wide array of emotional and social challenges associated with end-of-life issues. Your skills in empathetic communication and crisis intervention will be vital in helping families cope with the complexities of grief and loss while navigating healthcare decisions.

Qualifications
  • A Masterโ€™s or Doctorate in Social Work (MSW or DSW) from an accredited program is required.
  • Current licensure from the State Department of Public Health as a Licensed Clinical Social Worker (LCSW).
  • Minimum of one year of clinical experience, preferably in a hospice or palliative care setting.
  • Valid driver's license, as occasional travel to meet with families may be required.
  • Strong organizational and time management skills to handle a caseload effectively and efficiently.
  • Demonstrated ability to communicate effectively with diverse populations.

Preferred Experience
