The role is primarily remote, offering flexibility while providing the essential services required by our senior clients. You will work from the comfort of your home but engage regularly with a multidisciplinary team, including healthcare professionals, caregivers, and family members. • Conduct thorough psychosocial assessments for senior clients, evaluating emotional, mental, and social factors that influence their well-being.
• Collaborate closely with clients and their families to create individualized care plans aimed at fulfilling specific health and personal goals.
• Provide counseling support during significant life transitions, such as health changes, moving to assisted living, or loss of loved ones, to foster resilience.
• Advocate for clients’ rights and access to essential community services, ensuring they receive the support they need to maintain their dignity and quality of life.
• Work in conjunction with interdisciplinary teams to discuss care strategies, share insights from assessments, and resolve any client-related challenges collaboratively.
• Conduct regular check-ins via video conferences to monitor client well-being, assess living conditions, and proactively address any care issues.
• Offer educational resources and information about community programs that promote social engagement and enhance clients’ quality of life.
• Maintain precise documentation regarding client interactions, assessments, care plans, and progress notes to comply with legal and ethical standards.

Your primary focus will be on senior adults who may face various challenges related to aging, including mental health issues, emotional distress, and social isolation. Understanding the nuances of geriatric social work will be pivotal, as you navigate client dynamics and family issues that arise during such sensitive periods in their lives.

• A Master's degree in Social Work from an accredited institution is mandatory.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of hands-on experience in social work is required, with a preference for experience in geriatric or community settings.
• Strong grasp of social work ethics, principles, and current practices, particularly those relating to aging and community resources.
