• Deliver therapeutic services, including counseling and psychotherapy to hospice patients, ensuring that treatments align with their needs and preferences.
• Prepare detailed treatment and discharge plans, utilizing a holistic approach to facilitate clients’ mental, emotional, and spiritual well-being.
• Conduct thorough patient assessments through interviews and case history analysis to develop individualized therapeutic strategies.
• Analyze client data to identify underlying issues, determining the necessary course of therapy or referral for specialized care.
• Complete and submit all client documentation within 24 hours after service delivery to maintain compliance and proper records.
• Participate in mandatory training and professional development sessions to stay current in the field of social work and integrative therapies.

In this role, you will primarily work with hospice patients who face unique emotional and psychological challenges associated with end-of-life transitions. Your efforts will be particularly focused on providing empathetic support, addressing grief, and facilitating meaningful conversations around mental health and holistic healing practices. This encompasses incorporating non-traditional strategies such as meditation, breathwork, and other methods aligned with our clinic's philosophy.

• Required: Active LCSW licensure or degree in a closely related field.
• Educational background including a Master’s or Doctorate in Social Work.
• Current malpractice insurance and the ability to become credentialed on insurance panels.
• A minimum commitment of completing 10 sessions per week (each approximately 53 minutes) as part of your caseload.
• Proficient in utilizing digital EHR platforms to manage patient information and treatment plans.

• Demonstrable expertise in mental health counseling methods and techniques.
• Familiarity with clinical counseling principles, along with knowledge of treatment methodologies.
• Understanding of the legal and ethical issues surrounding patient rights and mental health care.
• A strong commitment to a holistic approach to mental health, including the integration of non-traditional therapeutic practices.
• Advanced technological skills related to delivering telehealth services effectively.

The annual salary range for this position is competitive, set at USD 90,000 to 105,000, with actual compensation determined based on your experience, geographic location, and unique skill set.
