Key Responsibilities- Actively coordinate with community resources to assist patients and families in accessing necessary behavioral health and environmental resources.
- Conduct comprehensive education for patients and their families on discharge options, considering mental health, legal, and insurance factors to promote informed decision-making.
- Provide continuity of care by offering referrals to ongoing community services and support upon discharge, ensuring that transitional care is seamless and effective.
- Schedule follow-up appointments for patients with psychiatric providers within a week post-discharge, and deliver essential discharge information to facilitate continued care.
- Develop detailed safety plans in collaboration with patients and families, incorporating preventative measures such as gun removal when appropriate, to enhance safety during challenging periods.
- Serve as a key communicator among healthcare teams, and external entities such as court officers and patient advocates, to expedite patient progress while respecting patient rights and facility safety protocols.
The Licensed Marriage and Family Therapist will work primarily with individuals and families experiencing a variety of challenges, including anxiety, depression, relationship issues, trauma, and life transitions. This role is critical in providing a safe and supportive atmosphere where clients can explore their thoughts and feelings, develop coping strategies, and work towards improving their emotional health and relational dynamics.
Qualifications- A valid and active state license as a Licensed Marriage and Family Therapist (LMFT) is required.
- A master’s degree in Marriage and Family Therapy or a closely related field is essential for this role.
- Minimum of 2 years of professional experience in a similar capacity, providing therapeutic services to individuals and families.
- Experience working in outpatient settings, with a strong understanding of the dynamics involved in community-based mental health care.
- Proficiency in utilizing electronic health record (EHR) systems for documentation and tracking patient progress.
- Familiarity with evidence-based therapeutic modalities, including cognitive behavioral therapy (CBT) and family systems therapy.
This position offers a competitive salary ranging from USD 120,000 to 130,000 per year, commensurate with experience and qualifications. Additional benefits may include:
