• Conduct comprehensive initial assessments of patients to understand their mental health needs, utilizing structured interviews, clinical observations, and psychological assessments.
• Design and implement individualized treatment plans that incorporate evidence-based therapeutic modalities tailored to meet the unique goals and needs of each patient.
• Provide a range of therapeutic services including individual, couples, family, and group therapy sessions, utilizing skills to create a safe and supportive environment for all participants.
• Diagnose mental health conditions and regularly monitor patient progress, adjusting treatment plans as necessary to ensure optimal outcomes.
• Provide crisis intervention services and coordinate referrals to emergency services as needed, ensuring timely assistance for patients in critical situations.
• Maintain accurate and up-to-date clinical documentation according to state and federal regulations, ensuring compliance and quality assurance within the organization.
• Collaborate effectively with other mental health professionals, medical providers, and community organizations to coordinate comprehensive care plans.

The LCSW will primarily work with individuals and families experiencing complex emotional responses to serious illness and the hospice care process. • A Master's degree in Social Work from an accredited institution.
• Valid and active LCSW license from the state of California is required.
