• Conduct comprehensive clinical assessments to evaluate patients' emotional, psychological, and social needs.
• Develop individualized treatment plans tailored to patients’ unique circumstances, ensuring integration with their medical care.
• Administer evidence-based therapeutic interventions such as grief counseling and crisis intervention strategies.
• Maintain accurate and timely clinical records in our electronic health record (EHR) system, ensuring compliance with all regulatory and organizational standards.
• Collaborate with a multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals, to ensure comprehensive patient care.
• Provide education and resources to patients and their families regarding available support services, coping strategies, and community resources.
• Stay updated on current research and developments in hospice care and mental health best practices to continuously improve the care provided to our patients.

You will work with a diverse group of patients and families, including individuals from various cultural backgrounds facing terminal illnesses. Your work will focus on addressing psychological distress, facilitating end-of-life planning, and providing bereavement support. The insights you offer will play a crucial role in enhancing the quality of life for patients in the hospice program and their loved ones, making your contributions vital to their well-being during challenging times.

• A Master's degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Michigan.
