Heartfelt Connections Therapy is seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our dedicated team in Pasadena, Texas. This position is fully remote, providing flexibility to tailor work hours to accommodate client needs while ensuring optimal care and service delivery. Our work hours typically span Monday through Friday from 8:00 AM to 4:30 PM, with the possibility of occasional evenings or weekends depending on client requirements. On-call availability may be needed to respond to urgent situations or support clients during off-hours, emphasizing the commitment to flexible and responsive service.

As an LCSW at Heartfelt Connections Therapy, you will engage in a variety of essential tasks, including:

• Conducting comprehensive assessments of clients' psychosocial needs, considering factors such as emotional health, mental well-being, and social circumstances.
• Working collaboratively with seniors and their families to create tailored care plans that reflect their personal goals and preferences.
• Providing individual and family counseling to assist clients in navigating life transitions, managing challenges, and strengthening resilience.
• Advocating for clients’ rights, helping them access critical community resources, and ensuring they receive necessary services.
• Engaging with an interdisciplinary team that includes nurses, therapists, and healthcare providers to coordinate comprehensive care plans effectively.
• Conducting regular follow-up visits to monitor clients’ conditions, evaluate care effectiveness, and address any safety concerns.
• Facilitating communication among family members to enhance cooperation and support within the client's care process.
• Educating clients and their families on available resources, support groups, and community programs aimed at boosting social engagement and well-being.
• Maintaining accurate electronic documentation of assessments, treatment plans, progress notes, and other relevant interactions.
• Staying informed about current best practices in social work and ensuring strict adherence to ethical standards and regulatory compliance.

Your primary focus will be on seniors who often face significant life changes including health issues, loss of independence, or bereavement. Many of these clients require tailored emotional support to address their unique psychosocial challenges. By employing empathy and active listening skills, you will help clients navigate through these difficult times, enhancing their quality of life and promoting mental health stability.

• A Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• At least two years of experience in social work, preferably within a community health center or geriatric context.
• In-depth knowledge of social work principles, aging-related issues, and accessible community services.
• Exceptional assessment, counseling, and problem-solving abilities, with a strong emphasis on empathy and compassion.
• Excellent communication and interpersonal skills, facilitating effective collaboration with diverse teams.
• Proficiency in electronic health record (EHR) systems and other relevant technology applications.

• Experience in coordinating care for clients with complex psychosocial needs.
• Familiarity with community resources specific to the senior population in Pasadena, Texas.
• Relevant certifications in specialized areas of social work, such as gerontology or mental health.
