Position Overview

Social And Family Consulting is seeking a dedicated Licensed Professional Counselor (LPC) to join our compassionate and dynamic team in Salem, Oregon. Key Responsibilities
  • Conduct comprehensive assessments to understand the unique needs of each patient, forming the basis for tailored treatment plans.
  • Engage in clear and continual communication with supervisors, direct care staff, family members, and other stakeholders to discuss patient progress and adapt strategies as necessary.
  • Develop and present a tentative schedule for appointments, home visits, and observations to ensure the efficient monitoring of treatment plans.
  • Coordinate and conduct home visits aimed at obtaining vital insights through in-home observations, chart reviews, and staff interviews.
  • Ensure the effective implementation of individualized program plans, employing evidence-based practices to facilitate patient growth and success.
  • Participate actively in Interdisciplinary Team Meetings, contributing insights that inform collective care strategies.
  • Complete detailed monthly and quarterly progress notes to document patient advancements and setbacks.
  • Organize training sessions for team members to ensure proficiency in implementing treatment plans and maintaining accurate documentation.
  • Conduct follow-up interviews and ongoing assessments to provide timely support for clients' evolving needs.
  • Maintain meticulous records to guarantee transparency and accessibility of patient information for relevant parties.
  • Act as a liaison with the public and the mental health community to facilitate development goals and enhance community awareness.
  • Contribute to the development of training materials aimed at optimizing our approach and improving outcomes for our clients.
Patient Population / Client Focus

You will work with a diverse array of clients, including but not limited to children, adolescents, adults, and families facing varying challenges such as trauma, substance misuse, and relational issues. Your expertise will play a vital role in promoting the emotional and psychological wellness of our community.

Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
  • An active and unrestricted professional counseling license in the state of Oregon.
  • A minimum of 2 years of counseling experience, ideally within a clinical context.
  • In-depth understanding of legal and ethical standards pertaining to mental health practice.
  • Exceptional interpersonal and communication skills, with the ability to establish rapport with a wide range of clients.
  • Proven ability to work collaboratively within a multidisciplinary healthcare team.
  • Familiarity with EMR software and other relevant technological tools to enhance documentation and patient care.
Preferred Experience
  • Experience or specialization in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is highly preferred and valued.
  • Experience in community outreach programs or developing training initiatives within a mental health setting.
