Key Responsibilities
  • Conduct ongoing assessments to identify and address the specific needs of clients, utilizing evidence-based practices tailored to individual circumstances.
  • Facilitate clear communication with supervisors, family members, and other stakeholders regarding the progress of clients, thereby fostering a collaborative treatment approach.
  • Organize and manage a tentative schedule of appointments, home visits, and observations, ensuring timely follow-ups and progress tracking.
  • Coordinate home visits when necessary, conducting detailed chart reviews and staff interviews while analyzing data to adjust treatment plans as required.
  • Implement and monitor program plans effectively, adjusting strategies to ensure optimal client engagement and outcome achievement.
  • Actively participate in interdisciplinary team meetings to align strategies and share insights, thereby enriching the collective understanding and support for each client.
  • Document monthly and quarterly progress notes reflecting client advancements, areas of concern, and recommended adjustments to treatment protocols.
  • Facilitate training for staff to ensure proficiency in program plan implementation and compliance with documentation standards.
  • Conduct follow-up interviews and additional home visits to provide ongoing support and resources to aid clients in their recovery journey.
  • Maintain meticulous records to facilitate information sharing with approved parties and uphold ethical standards in client confidentiality.
  • Act as a liaison for clients within the public and mental health communities, supporting their development goals and resource navigation.
