Job Description:
Job Summary
Responsible for providing disease-specific care and education to patients with acute or chronic conditions in a busy ambulatory setting. Conducts skilled assessments, develops and implements individualized care plans, and supports patients and families in navigating healthcare services. Acts as a liaison between patients, medical staff, and community resources. Administers intravenous infusions as needed. Maintains advanced specialty training to serve as the nurse expert for the multidisciplinary team, ensuring , evidence-based care.
Essential Functions
• Assesses and reassesses patients using age- and culturally-appropriate criteria, considering physical, psychological, emotional, spiritual, cultural, and environmental needs.
• Identifies patient needs and nursing diagnoses, formulates and documents individualized plans of care in collaboration with patients, families, and the multidisciplinary team.
• Updates and documents plans of care based on ongoing assessments and patient responses, administering medications safely and accurately according to policy.
• Performs nursing procedures and interventions safely, ensuring thorough and timely documentation in accordance with hospital policies.
• Coordinates discharge planning and provides patient and family education throughout hospitalization, evaluating and modifying care as needed.
• Recognizes and responds to changes in patient status, promptly notifying physicians and other healthcare professionals as appropriate.
• Collaborates with interdisciplinary teams to improve care processes, outcomes, and patient satisfaction, fostering a culture of teamwork and service excellence.
• Maintains required licensure, specialty training, certifications, and participates in ongoing professional development and staff meetings.
• Engages in performance improvement initiatives, data collection, and process optimization to support quality and safety goals.
• Performs other duties as assigned.
Education Requirement
Associate's degree in nursing
Bachelor's degree in nursing (BSN) is preferred
Experience Requirement
0-2 years of experience in nursing, or a relevant area
An equivalent combination of education and experience may be considered.
All experience must be paid and in the same related field.
