As a vital contributor to our patients’ well-being, you will engage directly with clients experiencing symptoms of severe emotional disorders and major mental illnesses, providing therapeutic support to enhance their quality of life.

Work Environment

This role is entirely remote, offering you the flexibility to work from the comfort of your home while still being able to positively impact the lives of our clients at our affiliated Hospice Center located in Elgin, Illinois. The position requires a full-time commitment of Monday through Friday from 8:00 AM to 4:30 PM, with occasional evenings or weekends as needed to address client availability and requirements.

  • Conduct comprehensive assessments and develop individualized treatment plans tailored to meet the unique needs of each client, focusing on their mental health challenges and personal goals.
  • Facilitate individual and group therapy sessions, employing evidence-based therapeutic techniques to foster coping strategies and resilience in clients facing mental health crises.
  • Collaborate effectively with a multidisciplinary team, ensuring the seamless integration of care by sharing insights and strategies aimed at achieving common objectives focused on patient empowerment and recovery.
  • Maintain meticulous records of client progress and treatment outcomes, adhering to ethical guidelines and organizational policies while using the electronic health records (EHR) system effectively.

Patient Population / Client Focus

This role focuses on clients who are experiencing major mental illnesses and severe emotional disorders, many of whom are facing complex life transitions, particularly those in hospice care. Qualifications
  • Must hold a current and valid LCSW license.
  • Bachelor's degree in social work or a related field.
