Job description

UMass Memorial Health is seeking an experienced and innovative Advanced Practice Provider (APP) to join our growing Advanced Therapeutics Program. This is a per diem, fully remote position where you will play a key role in managing patients with obesity, diabetes, and cardiometabolic conditions through telehealth.

Given the innovative and evolving nature of this program, we are seeking candidates with at least 2+ years of relevant provider clinical experience and a strong interest in virtual care delivery, care model development, and chronic medication management. This is an excellent opportunity for providers who value clinical autonomy, collaboration, and contributing to the development of new care pathways.

Key Highlights:

  • Conduct comprehensive telehealth visits for patients with obesity, diabetes, and cardiometabolic conditions

  • Initiate and manage advanced therapeutics (GLP-1s, dual incretin therapies, oral weight management agents, SGLT2 inhibitors, PCSK9 inhibitors, etc.)

  • Monitor laboratory results, adjust medications, and ensure safe, effective treatment plans

  • Educate and empower patients on lifestyle modification, disease management, and medication adherence

  • Collaborate with a multidisciplinary care team, including physicians, pharmacists, dietitians, and behavioral health specialists

  • Support patients with medication access, including prior authorizations and adherence strategies

  • Utilize analytics and patient data to track outcomes and optimize care plans

  • Contribute to program development, workflow optimization, and quality improvement initiatives

Schedule:

  • Per diem (as needed)

  • Variable 4 to 8-hour shifts between 7am to 5pm

  • 100% remote

  • M-F, No weekends or holidays

Qualifications

Required:

  • Current Massachusetts NP or PA licensure and Master s degree

  • Certification from an accredited Nurse Practitioner or Physician Assistant program

  • BLS and ACLS certification

  • Minimum of 2+ years of experience in an APP role managing chronic conditions (obesity, diabetes, cardiometabolic disease)

  • Experience prescribing and managing advanced therapeutics (GLP-1s, SGLT2 inhibitors, PCSK9 inhibitors, weight management medications)

  • Comfort working in a fully virtual care environment

  • Strong clinical judgment, communication, and critical thinking skills

  • Ability to work both independently and collaboratively in a multidisciplinary team

Preferred:

  • Experience with Epic EMR

  • Experience in telehealth or digital health environments

  • Interest in population health, preventive care, and value-based care models
