About Us
This temporary position will be supporting The Queen's Health Systems' Remote Patient Monitoring Program (RPM), which launched March 2025. The RPM program is in partnership between The Queen's Health Systems' Native Hawaiian Health Department and Health Recovery Solutions (HRS). This program's renewal is dependent on grant funding.

This is a community-based position with a hybrid work schedule.
Vision Statement
To innovate the care experience by implementing workflows, EHR tools and RPM devices that will help engage Native Hawaiians and all the people of Hawaii in their own healthcare and wellbeing and help them to build lifetime partners in health with the care teams that support them.
Purpose Statement
The initial focus of the Remote Patient Monitoring program will be on the Native Hawaiian population with goals of: Closing the life expectancy gap by half through better management of chronic diseases to slow the progression of Diabetes, Hypertension, CHF, COPD, and Obesity. This solution will be compatible with multiple devices and fully integrate into the Epic EMR from both the patient and provider perspective. We will then expand the RPM solution to the entire patient population across the Queen's Health System, including ICF, SNF, and Hospital at Home models. This will allow us to reduce readmissions and improve clinical outcomes. RPM will increase clinical impact by improving Epic integration and ease of use for patients. We will support patients in real-time and meet our kina'ole goal to treat the right patient, at the right time, in the right location, in the right way.
Responsibilities
I. JOB SUMMARY/RESPONSIBILITIES:
• Supports community health by establishing and maintaining trusting relationships with patients, families, and providers to promote health, recovery, resiliency, and wellness.
• Performs activities off-site, in the community, and makes patient home visits as required.
• Performs duties and responsibilities in accordance with policies and procedures and consistent with the goals and objective of the department and the Queen's University Medical Group (QUMG).
II. TYPICAL PHYSICAL DEMANDS:
• Essential: stooping/bending, finger dexterity, seeing, hearing, speaking, lifting usual weight of 10 pounds up to 25 pounds.
• Frequent: standing, walking, reaching at and below shoulder level.
• Occasional: sitting, kneeling, climbing stairs, walking on uneven ground, squatting, pushing/pulling usual weight of 1 pounds up to 2.5 pounds of force, carrying usual weight of 5 pounds, reaching above shoulder level.
III. TYPICAL WORKING CONDITIONS:
• Not substantially subjected to adverse environmental conditions.
• Work involves extensive off-site activities, including home and community-based visits with patients.
• Must have reliable transportation in order to make visits to sites where patients reside.
• May be exposed to negative responses from patient or family.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
• High School diploma or equivalent.
• Valid U.S. driver's license required in accordance with Hawaii state law rules and regulations.
• Driver's abstract or evidence of good driving record.
• Medical Assistant, Community Health Worker or Patient Navigation certification preferred.
• Current BLS certification.
B. EXPERIENCE:
• Two (2) years of relevant healthcare/social services experience; or one (1) year of healthcare/social services experience with a Community Health Worker certificate.
• Previous experience working with individuals with behavioral health and/or substance use issues preferred.
• Prior experience working with Native Hawaiian and other underserved populations preferred.
