Work Environment

As a full-time employee, you will typically work Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening and weekend hours as dictated by client needs. Key Responsibilities
  • Evaluate the psychosocial needs of seniors through comprehensive assessments that take into account emotional, mental, and social factors.
  • Collaborate with clients and their families to create personalized care plans tailored to individual goals and circumstances.
  • Provide counseling and emotional support, guiding seniors and their families through life transitions such as health crises, grief, and relocation.
  • Advocate for clients' rights and help them access necessary community resources, ensuring they receive the appropriate support tailored to their situations.
  • Work alongside an interdisciplinary team composed of nurses, caregivers, and various healthcare professionals to ensure a holistic approach to care.
  • Conduct regular check-ins with clients to monitor their well-being and living conditions, addressing any safety concerns and adapting plans as necessary.
  • Facilitate communication among family members involved in the caregiving process, resolving conflicts and promoting effective team collaboration.
  • Educate clients and their families about available community resources, support groups, and programs designed to improve social engagement and the overall quality of life.
  • Maintain meticulous documentation of assessments, care plans, and interactions in compliance with agency standards and ethical practices.
  • Stay updated on current social work practices, tools, and regulations to ensure quality service delivery and ethical adherence.
Patient Population / Client Focus

Your primary focus will be on the senior population, who often face unique emotional and psychological challenges. This role requires a deep understanding of aging-related issues, including coping with chronic illness, isolation, and loss. By developing trusting relationships and demonstrating empathy, you will empower clients and their families to navigate their concerns and regain a sense of control over their lives.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • Current state licensure as a Licensed Clinical Social Worker (LCSW).
  • A minimum of two years of experience in a social work role, specifically within a community health or geriatric setting.
  • Comprehensive knowledge of social work principles, aging-related challenges, and community resources available to clients.
  • Strong assessment, counseling, and problem-solving skills tailored to meet the needs of seniors.
  • Exceptional communication and interpersonal skills, facilitating effective collaboration with clients, families, and interdisciplinary teams.
