PHYSICAL THERAPIST (PT) – HOME HEALTH | PER DIEM 1099

About the Role:
Unlimited Health Care Services Inc. is looking for a compassionate and skilled Physical Therapist (PT) to join our home health team in Fort Lauderdale and East Broward County.  Our patients are all from Deerfield Beach to Broward Blvd east Broward.
This is a per diem, 1099 opportunity designed for PTs who value flexibility and want greater control over their schedules. Whether you're looking for additional income, want to work around an existing schedule, or simply prefer the independence of providing one-on-one care in the home, this position gives you the flexibility to choose visits that work for you while maintaining a healthy work-life balance.
As a Home Health PT, you'll work directly with patients in their homes to help improve mobility, strength, balance, function, and independence. You'll have the opportunity to develop meaningful relationships with your patients while making a measurable difference in their quality of life.

Responsibilities:
  • Perform comprehensive physical therapy evaluations and assessments
  • Develop individualized treatment plans based on each patient's needs and goals
  • Provide skilled physical therapy interventions in the home setting
  • Implement therapeutic exercises and activities to improve strength, mobility, balance, coordination, and functional independence
  • Evaluate patient progress and modify treatment plans as appropriate
  • Educate patients and caregivers on therapeutic exercises, mobility, positioning, safety, and home exercise programs
  • Instruct patients and caregivers in techniques to promote safe mobility and independence
  • Communicate patient progress and changes in condition with physicians, nurses, and other members of the care team
  • Complete accurate and timely clinical documentation for each visit
  • Participate as an active member of the interdisciplinary care team
Requirements:
  • Active Florida Physical Therapist (PT) license in good standing
  • Doctor of Physical Therapy (DPT) from an accredited program
  • Current CPR/BLS certification
  • Strong clinical assessment and problem-solving skills
  • Excellent communication and interpersonal skills
  • Valid driver's license and reliable transportation for patient home visits
  • Current auto insurance
  • Ability to work independently and manage your visits responsibly
  • Compassionate, patient-centered approach to care
Compensation:
  • Routine Visit: $80 per visit

  • Evaluation: $90–$100 per visit
