Pharmacist (Hawaii - Remote)

Target Location: Hawaii (HI)

Licensure Requirement: Active pharmacist license in Hawaii required


Role Overview

A forward-thinking telehealth organization is seeking a Pharmacist to support patients across Hawaii through remote clinical services.

This position is centered on virtual care delivery, with a strong emphasis on medication optimization, patient education, and collaboration with interdisciplinary care teams to improve health outcomes.

This role is ideal for pharmacists who are adaptable, patient-focused, and confident in leveraging digital tools to deliver high-quality care.


Key Responsibilities

  • Provide comprehensive Medication Therapy Management (MTM) services
  • Conduct medication reconciliation and assess therapy effectiveness
  • Deliver patient-focused education on medications and chronic conditions
  • Support chronic disease management (e.g., diabetes, hypertension, COPD, CHF)
  • Utilize motivational interviewing techniques to drive adherence and engagement
  • Collaborate with providers to optimize medication regimens and care plans

Required Qualifications

  • Doctor of Pharmacy (PharmD)
