*This is a full-time, remote opportunity working Monday-Friday, 8am-5pm. Must reside in Maricopa or Pinal County.*

The Workflow Specialist (RN) is responsible for coordinating and managing key clinical and operational processes to ensure efficient home health services and regulatory compliance. This role serves as a central resource for plan of care, orders, OASIS review, care coordination, documentation oversight, and workflow optimization. The Workflow Specialist partners closely with clinical leadership, field staff, physicians, referral sources, and support teams to support timely patient admissions, high-quality care delivery, and positive patient outcomes.

Responsibilities

  • Coordinate and oversee daily clinical workflow processes to ensure efficient home health operations
  • Review and suggest edits on all home health OASIS data, home health care plan required compliance items, and analyze quality outcome data for assessment and improvement of agency performance in star rating and value based purchasing methodology
  • Collaborate with clinicians, Clinical Managers, and operational staff to coordinate patient care and scheduling needs
  • Review clinical documentation for accuracy, completeness, and compliance with Medicare, state, and organizational requirements
  • Monitor physician orders, certifications, recertifications, and other required documentation to ensure timely completion
  • Provide clinical support and guidance regarding documentation standards, regulatory requirements, and workflow processes
  • Assist in managing workflow queues, dashboards, and worklists within the electronic medical record (EMR) system
  • Serve as a liaison between field clinicians, physicians, referral sources, and internal departments to facilitate effective communication and care coordination
  • Identify workflow inefficiencies and recommend process improvements to enhance operational effectiveness and patient care
  • Support quality assurance and performance improvement initiatives through ongoing monitoring and auditing activities
  • Track and report key operational and clinical performance indicators as requested
  • Assist with staff education and training regarding clinical documentation, compliance requirements, and workflow processes
  • Maintain confidentiality and protect patient information in accordance with HIPAA regulations

Requirements

  • Current, unrestricted Registered Nurse (RN) license in the State of Arizona required
  • Associate or Bachelor's degree in Nursing required
  • Minimum of one (1) year of home health, hospice, case management, or healthcare operations experience preferred
  • Knowledge of Medicare Conditions of Participation, home health regulations, and documentation requirements
  • Experience with electronic medical record (EMR) systems and healthcare technology platforms
  • Strong clinical assessment, organizational, and problem-solving skills
  • Excellent verbal and written communication skills
  • Ability to manage multiple priorities in a fast-paced healthcare environment
