Work Environment

This full-time position offers the convenience of remote work, allowing you to provide essential services from your own space while maintaining the resources of a well-equipped hospice center. Key Responsibilities
  • Conduct thorough assessments to develop personalized treatment plans that address individual mental health needs.
  • Utilize evidence-based interventions and therapeutic techniques to promote client well-being and facilitate goal attainment.
  • Maintain accurate and confidential clinical records, ensuring all documentation meets compliance standards and best practices.
  • Collaborate with an interdisciplinary treatment team to deliver holistic and integrated care for our patients.

Patient Population / Client Focus

Qualifications
  • A Master's degree in Social Work (MSW) from an accredited institution is required.
  • A valid LCSW license issued in the state of New York.
