• Conduct comprehensive psychosocial assessments of patients to identify emotional, social, and financial needs, utilizing clinical tools and methodologies.
• Provide individual and group counseling to patients, focusing specifically on adjustment issues, grief, coping mechanisms, and resilience building.
• Collaborate closely with interdisciplinary medical teams to formulate holistic treatment plans that address both physical and psychosocial needs of patients.
• Develop and implement personalized discharge plans, coordinating with community organizations and resources to ensure continuity of care post-discharge.
• Offer emotional support and education on treatment options to patients and their families, guiding them through the complexities of recovery and support resources.
• Maintain accurate, timely documentation in compliance with hospital policies and legal requirements to facilitate patient care and improve organizational efficiency.
• Participate actively in hospital committees, providing insights and expertise on social work-related matters to enhance service delivery and clinical quality.

Your primary focus will be on patients experiencing substance use disorders, including those who are in the detoxification process. You will work with diverse individuals and families, helping them navigate the challenges associated with addiction recovery. Your empathetic approach and clinical expertise will help foster a supportive therapeutic environment conducive to healing and resilience.

• Master's degree in Social Work from an accredited institution.
• Current active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of clinical experience in a healthcare or hospital setting, preferably in behavioral health.
• Demonstrated knowledge of medical terminology and mental health disorders, especially in the context of substance use.
• Proficiency in crisis intervention techniques and client assessment methodologies.
• Strong capabilities in communication, collaboration, and interpersonal interactions.
